For students under 18 years of age:


      1)  To participate in the field trip, you will need to obtain
          a parent's or a guardian's signature on the Participant
          Agreement form.

      2)  To make sure this gets done in time for the September 17
          trip, the best way to do this will be the FAX the form
          to your parent or guardian, get the signature, and have
          the parent or guardian return it by FAX.

      3)  This will only work, though, if you can obtain a FAX
          number from your parent or guardian to be used for the 
          exchange of forms. 

      4)  When you have identified such a number with your parent
          or guardian, *or if you need to make alternate arrangements
          to get the signature*, Ms. Faye Caouette, the Montserrat Program 
          Coordinator, can help you with all steps of this process, and
          you will be using the FAX machine in her office on this end.
