New release (v0.8.9rc3)

As pointed out by Robin Blume-Kohout, there's a (potential) conflict with
the "fill;" macro and part of the C++ STL, at least with gcc-3.2. To avoid
future problems, several macros introduced in Version 0.8.5 have been
reverted to function calls, and macros will be phased out of the source
code in general.

A few tweaks were made to Pedersen's contrib package; the explicit use of
std:: has been removed (R. B-K again:) and a few ints have been replaced
by bools or enums.

Finally, the manual has been updated to 0.8.9; much of the discussion
about customization was obsolete...

Apologies for any inconvenience caused by the introduction (and
withdrawal) of color and style macros in 0.8.5; it was dictated by pure
laziness (does anything else motivate a mathematician?:) but in retrospect
was a bad design decision.

The update_figs.sh script automates the process of reverting input files
that need to be re-processed, and has more options and fail-safe measures
than the earlier version: It will not overwrite files without asking for
confirmation, and can process files given on the command line. (If no
files are specified, the script processes all .xp, .c, .cc, and .C files
in the current directory.) The script successfully reverted the sample
files and the figures in the tutorial.
